Post-Hospital Recovery

The step between hospital and home.

Short-term recovery support for people who are medically ready to leave hospital but not yet ready, confident or practically able to return home — after joint replacement, a fracture, a fall, or a hospital stay that took more strength than expected.

Transitional Living & Reablement

Independence, rebuilt in a space of your own.

Time-limited support in a spacious, self-contained apartment — for people awaiting adaptations to their home, rebuilding daily-living skills, or needing a short period of support before returning to fully independent living.

Residential & respite care

Round-the-clock reassurance in the main house — for planned respite, and for guests whose assessment shows a residential setting suits them best.

Transitional & independent living

Spacious, self-contained apartments — your own front door, with an assessed level of care and support arranged around you.

Every guest joins the same Hospital to Home pathway — assessment simply decides which setting, and which level of flexible support, is right.

Stay in one of our apartments and choose the level of support that feels right for you. For transitional stays, any personal care arranged through Pembroke is provided by our own CQC-registered service, offering continuity and reassurance throughout your recovery. For other stays, the choice remains yours: receive support from Pembroke, continue with a care provider you already know, or enjoy the apartment independently without care.
